| Billing Our clients may choose to be
billed directly or have their patients charged directly for our services. If patient
billing is elected, please be sure to notify your patients that they will be invoiced by
us. In addition, we accept third-party payments from most agencies. In general, we
can bill the third party directly for the primary charges and bill the patient for
additional charges not covered by his or her program. We encourage patients to send BCL
the insurance forms authorizing direct payment, which we submit for
reimbursement. Please note that certain HMO's require prior approval of
laboratory procedures.

Third-Party Billing Information
BCL can bill and will accept payment from many agencies and insurance
companies. Provide all the information to avoid follow-up correspondence from the
Billing Department.
Ask your patients to show you their identification cards to ensure proper ID number,
eligibility dates, correct spelling of names, etc. This information must appear
clearly on the requisition form. A copy of their card (front and back) attached to
the requisition is preferred. Patients will be responsible for such charges as
non-covered services, co-insurance charge, and deductible amounts depending on the type of
coverage they have. It is the patient's responsibility to collect from their own
insurance company, if the laboratory does not accept assignment. If you have any
